Program Innovation Award

Does your facility have an innovative program that should be celebrated? The Program Innovation award is given to a facility or individual who has designed the most innovative program that meets the following eligibility criteria:

• Facility or individual must be a current member of the MFA for at least one year 
• Program provides a needed service that benefits the membership or the community at large 
• Program addresses at least one or the following categories
  • Clinical Integration 
  • Member Retention 
  • Community Outreach 
  • Program has demonstrated growth over the past year(s) 
  • Program is reproducible
